The Basketball Factory Summer Camp DOES NOT provide health and accident insurance. Campers must rely on their guardian's medical insurance plan for payment of all medical services. PERTINENT INSURANCE INFORMATION MUST BE INCLUDED ON THE APPLICATION. Minor sports injuries are treated by staff trainers: serious injuries will be handled by local hospitals. The Basketball Factory Summer Camp waives all responsiblities for treatment of camp-related injuries.
